Home From Home Care wanted to revolutionise the way care was delivered to adults with learning disabilities. When they started in 2004, they looked around for other providers doing things differently, but they found very few examples to emulate and limited software solutions to support a new model of care. So, starting with a blank sheet of paper, they built their own from the ground-up.
Home From Home Care was effectively Zone Nation’s first client, with care and technical teams working together to develop a new data-informed model of care. Today, every part of Home From Home Care’s model is powered and informed by Zone – from workforce to scheduling, finance, property management, asset management, quality and risk decisions.
Zone’s all-in-one platform means that they can quickly anticipate, adapt and respond to an individual’s behaviours, goals and preferences. Colleagues access real-time data, management support and additional specialist resources, so the individual has the right support for their changing needs.
Data on an individual’s health, wellbeing and behaviours is, as appropriate, collected and analysed by dedicated specialist teams. Together with reactions to events, activities, preferences, diet and aspirations, the data enables Home From Home Care to spot changes in behaviour, identify issues, opportunities, and to adapt the individual’s service.
Home From Home Care can support managers and support workers to deliver great outcomes for each individual. It is integral to achieving appropriate staffing levels and colleagues with the right attributes for each individual, whilst also supporting the team with their own health and wellbeing.
